Hospital safety is naturally a huge concern, but how do you know that your health care facility is up to par? While some area hospitals have received a healthy score, some need significant improvements.

Leapfrog Hospital Safety Grade is a national rating system designed five years ago, which focuses on hospitals in the United States. The system looks at health care facilities all over the country and rates them based on errors, accidents and infections. It has helped to significantly improve many hospitals by holding them accountable for these issues.

However, while many hospitals have improved, Leapfrog predicts that over 1,000 people a day will die from mistakes that could be avoided. Hospital error is the third leading cause of death in America.

    Northern Westchester Medical Center

    Located at 400 East Main Street in Mount Kisco, NY, Northern Westchester Medical Center has maintained its "A" rating for five straight rating periods from Spring and Fall of 2015, Spring and Fall of 2016 and now Spring of 2017.

    This health facility continued to improve from a "C" in the Spring of 2014 to a B in the Fall of 2014.

    Griffin Hospital

    This is the sixth straight "A" for this Derby, CT hospital located at 130 Griffin Street, Derby, CT.

    Greenwich Hospital

    Dropped from the two "A" ratings they got in the Spring and Fall of 2016, this medical facility at 5 Perrywich Road in Greenwich, CT has still rated higher than the four straight "Cs" they received in 2015 and 2014.

    Stamford Health

    Another hospital that dropped from three straight "A's" is this facility located at 1 Hospital Plaza, Stamford, CT. Although their greatly improved from the "Cs" they earned in Spring and Fall of 2014 and Spring of 2015 they did not maintain the "A" rating they received in the Fall of 2015 and Spring and Fall of 2016.

    Vassar Brothers Medical Center

    Dropping from the "A" they received last Fall and after two consecutive Cs, this medical center at 45 Reade Place in Poughkeepsie, NY had three consecutive "A's" in 2014 and 2015.

    Bridgeport Hospital

    Spring 2017 gave the medical facility located at 27 Grant Street, Bridgeport, CT its third straight "C". Although it rose from a "D" in Fall of 2015, in the Spring of 2015 it received a "C" after consecutive "B's" in 2014.

    It looks like this medical center located at 24 Hospital Avenue in Danbury, CT is getting back in the game after a pretty bad 2016 where it scored two "D's". 2014 was a pretty good year for them garnering two "B's" and then going down to two "C's" in 2015.

    Located at 21 Elm Street in New Milford, CT, this area hospital seems to hold a consistent "C". No score were available for 2016 and half of 2015 but earlier scores in 2015 and 2014 hold steady at a "C".

    Located at 34 Maple Street, Norwalk, CT, Norwalk Hospital earns "C's" across the board for years 2014, 2015, 2016 and now Spring of 2017.

    Heading downhill is this medical center located at 670 Stoneleigh Avenue, Carmel, NY. Going from two "A" ratings in 2014, they slipped to a "B" rating and held that grade for four scoring periods in 2015 and 2016 going to a "C" in 2017.
